A MUST, No Matter Your Health or Background I like the compact style and easy storability. It fits in my wallet better than my last emergency information sheet. The pages are easy to write on and most of the sections are necessities. There is a lot of redundancy in terms of the emergency contacts and family history. For example, clearly my spouse would be my primary emergency contact (and it's probably that way for most couples), however, since there was nothing to indicate that the spouse should be the first point of contact, I found myself writing her information in the spouse section and in the emergency contact section, and again in the contact for information on my living will and organ donation wishes. I also would have liked to have seen "Tissue Donor" in addition to "Organ Donor" since many places treat these as separate consent forms. May 20, 2013
